Select the song from the dropdown that says "Äänestettävä kappale". If POTF isn't listed there, you can enter the song manually in the field that says "Tai oma valintasi" (="Your own choice").

Here's a brief translation of the other fields you'll need to fill in:
Nimi = Name
Ikä = Age
Osoite = Address
Postinumero ja -paikka = Zip and city
Puhelin = Phone
Lähetä = Send

Note! You don't actually have to write any real information in the fields, as long as you put something there...
